Could macOS allow apps things like screen recording and accessibility after the user allows it, without restarting? Most permissions can apply without doing so, I don't see why they can't either. Does it have anything to do with the way macOS was programmed or designed, or its APIs?

Strictly they are not permissions and the difference is the key.

Permissions are attached to objects and specify who/what/how can access that object.

Rights are authorisation to perform an operation that affects an entire computer rather than a specific object on the computer.

Yes, it has to do with the how applications gain rights. Somewhat roughly: At application startup an app discovers what rights it has. If it needs further rights all it can do is tell the user how to grant those rights to the application, quit and get the user to start it again. [A developer would be able to put that better.]

Contrary to what macOS tells you, you don't need to restart the app once the permission is approved. There are certain cases where an app has to be restarted, but most work just fine if you dismiss the restart app prompt. For example, Zoom can immediately share the screen once you approve the permission without having to restart.
