Go to setting > privacy > location services and see what is using the gps
 
The outline of the location services icon means something is geofenced. You've either got a location-based reminder or someone has added a geofenced notification to you in Find My Friends (which you can check).

Yeah that's the geofences icon. It annoys me to heck that its always there when an app uses that feature that I usually go in and turn off location for that app

Yup. Your phone is constantly using GPS to determine your location so it knows when you leave the geo-fenced area.

To save battery life I think Apple probably took the option to get location data from cell towers, not GPS. This way the battery is not drained.

The second option would be to use GPS. Those are the two methods that are used.
