If we can only order one watch per apple ID, do i and should i set up my wife to have her own apple id? We have about 6 devices all on my apple id so there was never a reason to set her up with one.

Yes, you should set up another Apple ID with a different email address for the purposes of ordering. Better to be prepared now than come 12am PDT when it's time to order.
