Not sure if this is the right place for this but this happened after updating to ios9 on iPhone.

This is my sisters personal experience since I'm keeping myself on 8.4. She recently updated her 6+ iphone to ios9 and not knowing all what's "new" to her phone didn't realize there was the option for "wifi assist" that is, by default, turned on. Long story short--If you don't have unlimited data be sure to turn wifi assist off. She only had a 3GB data limit and having this wifi assist automatically on she went over her data significantly. She's not much of a "tech person" so for most of us, this would be a no brainier to check all this out and be aware if you are on wifi or not.

Personally, I didn't know there was wifi assist & have unlimited data so I'm generally never concerned lol but I got the phone call hearing all about it (she says she's done with updating her iOS, she always has issues) so I am just posting this in case there is anyone else out there not aware.
