My 12 Pro max has some scratches on the screen that are nasty and they are bugging me. I have not put screen protector on my iPhones before because I take care of them. But iPhone 12 scratches so easy.

Anyhow. Some questions:
  • Is Apple able to repair the screen if it’s only scratches on it or does it need to be cracked?
  • Does Apple swap out the phone or only the screen?

I’m not asking about AppleCare, I’ve my own electronics insurance.

Depending on the insurance, if your policy pays for cosmetic damage then either they have the screen replaced by a 3rd party or possibly pay Apples out of warranty replacement cost and charge you a deductible. Apple won't replace the screen under warranty unless you pay the out of warranty replacement cost which is $329 U.S.

Typically scratches are not covered under warranty and you’d have to pay to get it replaced.

Apple will replace only the screen on the device as long as the housing isn’t severly damaged or bent. If it was then they replace the whole unit.
 
Wonder if a screen protector would hide it somewhat…
If you use one of those UV liquid protectors, like Dome glass, it would most definitely hide the scratches since they would be filled in. But I don't think a traditional screen protector would do that, I might be wrong though...
