I did this with the iPhone 5 (except I was switching from AT&T to Verizon).

When the phone arrived and I activated it, my phone number was automatically ported to Verizon and the phone worked. All I needed to do at that point was call up AT&T and terminate my service with them.

You can also change your plan when you receive your iPhone.

When my wife and I bought new iPhones at the same time, we couldn't sign up for a family plan during the pre-order process. The Verizon rep told us to just buy the phones with individual plans, and then call up Verizon to merge them.
