A while ago, I read that the iPhone's GPS can still be active even in airplane mode if the app that I'm using requires GPS. Have this been fixed?

Can an iPhone be used with a Bluetooth GPS? If so, how do I know that the iPhone is getting data from the Bluetooth GPS rather than from its internal GPS chip?
 
iPhone 6s has Bluetooth capability. any further compatibility and/or capability is up to developer of your external GPS unit - an entity for which none of us can answer without further information.

your unit's manufacturer could develop an app and allow it to only use data from the external unit, or specifically inform you when doing so. beyond that, I suppose it's impossible to know.
