Writing an iPhone web app

Just want to start with saying I'm a complete beginner at this .

For work I have to complete quite a few forms and would lik an easy to do to this on my iPhone .
I would like to write an app that would give me fields and choices and at the end export onto a PDF or document . I have the required documents in PDF already and can convert to .dox if needed .

Is there any easy way to do this ?
The document has lots off "tick boxes" and I'm not sure a PDF editor on the iPhone would be quick enough to complete onsite and email back to the office .

Or is there already an app to do what I'm lookin for ?
