Wrong pictures for bookmarks

hey guys ever since I can remember especially with iOS 9 when you click on the address bar it shows a list of your bookmarks like tiles.I always notice that another websites logo is on the wrong bookmark for example the CNN logo will be on MacRumors website for some strange reason. Are you guys seeing this and what can I do to solve the issue and yes I have set up my phone as brand new no iCloud restore

yes but the same happens on safari on my mac sometimes. i just learned to live with it ...
