i can be physically using the phone and it will often give zero indications of an incoming call, just a "missed call" notification. no ring, no vibrate, no visual indicators.

do not disturb is turned off. silence unknown callers is turned off. it can happen when i'm out, or when i'm at home on wifi (and wifi calling is enabled). no bluetooth devices are connected. i'm on ios 16.1

i'd estimate i miss 40% of calls as a result. this... doesn't seem right?

You don’t happen to have any kind of spam filter or call filter such as T-Mobile ScamShield or AT&T ActiveArmor, do you?
Nope. I’m in Canada on Fido. I guess there is an off chance it is a carrier issue?

There's always the potential. I would give your line settings a quick look on your carrier's app or website just to make sure you don't have anything enabled that could filter calls somehow. Never hurts to just rule out the possibility first.

i've never seen any sort of settings available in the fido app or website. they're a flanker brand so they probably have fewer bells and whistles. i also suspect its more a quality of service issue than a setting because while extremely frequent, its not consistent. some calls from both known and unknown numbers will come through, while many known and unknown calls will fail to arrive.

its especially disappointing at home where wifi calling should sidestep any reception issues, which made me wonder if it was something on the apple software side (a similar issue will happen where the phone suddenly stops connecting to the internet on cellular, but will immediately work again after a restart).
