Update!

Apple's site seems to work now, and the form looks a little different than the one emailed to mhorng, but I imagine either would work.

thanks! the apple one won't work for me still. just knocks me out to the order status page again.
 
It's still total BS that they don't offer the option to:

1. Pick up your phone at an Apple Store; or
2. Pick up your phone at the distribution center BEFORE a delivery attempt has been made.

UPS / FedEx home delivery is about the most inconvenient way to get something imaginable when you work for a living.
 
The apple link doesn't work for me yet either. I only got my tracking info yesterday though. I called apple and they said it takes about 24 hours to get into the system properly and you can print the form after that. If I can't print through apple later today, I'll just use the one here.
 
Anyone had success with this last year?

I didn't try it last year when I got my 4S, but I did try it for another apple.com order and my delivery guy completely ignored it even though it was taped to my front door at eye level. I wouldn't hold my breath if it doesn't work....

For what it's worth, I called Apple earlier today needing one of these for my iPhone 5 shipment and they emailed me the file attached here.

That's at least some good news. I plan to run out to pick up another 5 for my sister early in the morning, and I am going to leave this form filled out. I have a Verizon pre-order, not an Apple one though. We'll see, in the worst case I should be able to pick it up at the FedEx center.

Yep it's worth a shot. I'm doing it to. Simply because I don't know if I'll have time tomorrow night to go to the Fedex depot to pick up my phone, so if it works it works and if it doesn't well I'll probably just have to wait until Saturday to get it (not a big deal at all).
 
Just called VZW internet sales support (I ordered through Verizon) twice, and had two different agents tell me that I should write a free-form note stating that I request FedEx to leave the shipment at my door and that I relieve VZW and FedEx of all responsibility for the shipment. One of the agents said that tracking number is necessary, the other one said it wasn't but I could put it on if I wanted to. Both said to sign and date the note, and both said that they do not have a specific form available for this. I am going to call FedEx just to be sure, and I think I am going to go with a form from this thread myself.

Why in the world would they allow you to pick it up BEFORE delivery? If that were the case, then there would be a line around the block at the delivery centers at 8 am, and if you supposedly work, how do you have the time to get it at 8 am and not interfere with work, but not be home for its delivery during the day? If you want to pick it up you can on the day they attempt delivery. It sounds like you're just whining to try and get it earlier. IF you're at work all day, you can wait and pick it up after work. Since the delivery date is the 21, you're lucky they'll even let you pick it up without waiting till the next day (since technically delivery can take place anytime on the 21st.

Just calm down, call tomorrow morning and tell them you want to pick it up after work, and by 6 or 7 it'll probably be ready.

Remember, if you CHOSE to have it shipped (mind you they charged NOTHING to ship it from CHINA to your door 10,000 miles away), that was your choice. You're more than welcome to go stand in line at a store and try your luck, or have done in-store pick up and picked it up any time convenient for you.

So tired of seeing people complain about Apple when it was their own choice to order the way they did.

Just to save yourself some trouble, the directions given are what Fedex puts on their website for releasing packages for "indirect signatures". I looked it up. So yes that is what they require. With that said, that still doesn't mean the driver will drop it off. They might "over look it" when making the delivery. Simply put, if I was a driver and I was delivering a 650+ phone, I might "accidentally" overlook the release as well. S/he might rather get a possible butt chewing for "missing the note" than risk being accused of stealing should the phone I delivered go missing before the owner got to it.
 
FedEx pre-signing form?

To those who have/are planning to pre-sign for your package because you will be at work, is there some official FedEx form that you are using, or are you just scribbling your shipping number on a note with your signature?

Yes, anyone at the address. They're not checking ID, they're just transferring chain of custody, and anyone at your address can accept shipments from UPS or FedEx. . . hence why it's somewhat of an antiquated policy.
 
You would be a major idiot if you used a Pre-Sign Form for a product worth this amount of money . Anybody walking up or driving up could take your phone and you would have no way to get your money back . Think about this before you actually do something retarded !

Wow that was insightful, especially the way you ended the comment. Shows you're a well educated individual. Not everyone lives in an area where people steal from one another. Based on the way you ended your comment, I'm guessing you do.
