My '08 Mac Pro crashed today with odd screen artifacts and black and white flickering, as well as blue dots across the screen. restarted and it would not reboot. I would hear the chime, see the apple logo and the spinning wheel, then the screen would go to white with the blue dots and a few black patches.

I tried resetting the PRAM and the SMC, and neither worked. I am now in safe mode, but the blue dots are all over the screen, plus some thick vertical bars. Unplugging and re-plugging the video cable changed the vertacal bars from a garbled mess to simple gray lines, but the blue dots remain.

I assume this is a video card issue, but I can't google any similar problems. I found something from 2006 about "blue fuzzies" but its not the same problem that I have.

I have OSX 10.8.2 and the 8800GT in this 2.8ghz Quad core '08 Mac Pro. I am not sure where to go from here. Should I try reinstalling OSX? Should I replace the video card?

Here is the crash log I got after booting in Safe Mode.

The 8800GT is well known as a GPU that dies often...
If you are 100% sure that it is the GPU (which seems pretty obious, but go check), then there are tutorials on how to bake your GPU and it works again (yes, baking in the oven!)
    Google will help you there.

Before, try to put in another GPU to test if your GPU is really dying.

It's the memory. Pull 1/2 the memory restart, if still a problem put back in and pull other half restart. Still a problem then pull a stick at a time till you find the bad stcick

Yep, this is the culprit. Artifacts and log are enough confirmation.

OP, you need new graphics card or you can try to bake 8800 (google or youtube it).

@kendall: part of log which you have misinterpreted is only list of installed hardware (starting from "Model:"). Most important are here "Kernel extensions in backtrace".

Ultimate proof would be if system boots ok without extensions loaded (aka safe boot).

may as well try the bake first
