I have replaced Apple SSD in my Late 2015 27" 5K iMac about a year ago (last September). Used NGFF M.2 NVMe SSD adapter and ADATA XPG SX8200 Pro 1TB NVMe. Got great speeds (~3GB/s read/write) and no other issues.

About a month ago after returning from vacation I found my iMac stuck in Recovery mode. Not sure what happened (power outage or botched auto-update). It took few attempts to force it to boot, but eventually it has returned to normal.

Few days ago it started crashing with "3rd party NVMe controller. Command timeout. Write" in the crash log. Usually after the crash it get stuck in Recovery mode and disk utils not even listing SSD there. This is similar to the last month and I now believe the issue is similar. The bigger difference is now it happens all the time. It crashed several times while I was reading forums today. It also crashed few times when I tried to update to the latest macOs version. I am on 12.04 currently. I have nothing else running except browser, suddenly there is a beach ball and few seconds later black screen and boot to recovery. Turning power off/on helps with normal boot.

Did anyone else experience similar issues? Any ideas why it would go from being super stable for almost a year and becoming practically unusable?

nVME SSDs are not immortal. They do fail occasionally.
Or the adapter failed you.
Either way, you have to take them out to test, to be sure before replacing or sending to warranty.
