a) do we know, for sure, how we will activate the phone, if we ever get it
b) can it be used online if you have wireless at home
c) when are we likely to get confirmation
d) how much will it be to use full sat nav / get the app to do so?

Seems as if with O2 at least in the UK, you can activate at home using iTunes.

yes - if you order in store, it will all be done in store, with final sync for contacts etc at home
if you order online, the contract will be set up for you, and activate in itunes

b) can it be used online if you have wireless at home
yes, simply connect to the network, although you may not need to if you have unlimited data and 3g

c) when are we likely to get confirmation
confirmation of what?

d) how much will it be to use full sat nav / get the app to do so?
no idea, depends if an app is released and by who
