I think I've seen it once perhaps and possibly due to something weird on the web site (like some ad being weird or something else not loading properly, as happens in desktop browsers as well at times). So not really something general/widespread for whatever reason.I get this at least 50% of the time while browsing. iOS sure is buggy.

I've only seen this happen on The Verge, which is a very "heavy" website. Based on that I'd say it's more of a website issue, with some iOS bugs thrown in.
Though I don't get a message, Safari just goes blank when I switch over to a new tab.
