On the iPhone 23 latest software update, and even before that, Most calls I accept need me to press the green button and then I have to click a second join button. What’s the purpose of having a second button to press?

You're right. Why display the green "Accept" button at all, when pressing it doesn't actually answer the call.
In lock screen, a simple swipe is enough.

However, I solved it by selecting:
Settings > FaceTime > Incoming Calls > Full Screen
Incoming FaceTime calls are then announced in full screen instead of the banner and one single tap on "Accept" starts the conversation.
