I love the always-on screen, and would like to continue using it. But it does present some privacy concerns for me. Mostly I don’t like people being able to glance at my screen on my desk or car dock and see what videos, music or media I have been watching. For example if I watch a YouTube video, even if I stop the video and close the app it’s still on my Lock Screen. Before anyone makes the inevitable jokes about what I watching, no it’s not that. Nothing bad, I simply like keep my interests to myself, or it could be something NSFW or my kids to see.

The most annoying thing is the behaviour is unpredictable - sometimes the Lock Screen is empty, sometimes the controls and thumbnail are present, it’s almost random, or at-least seems that way.

Any way to fix this, or will I have to turn always on screen off?

I believe it’s not possible to do that yet, Apple doesn’t allow any customization for Always-On, and you can only turn it on or off. But I suggest you submit this suggestion to Apple, this is the kind of suggestion that makes a lot more sense now that we have always on display. Apple may add this feature in later iOS updates. Until then, you’re out of luck
 
For the large image/thumbnail, you can tap on that to minimize it into the media player/controls, and vice versa.
