I have the hourly reminders on just to get me to eat some fruit and be mindful - before apple had the sleep app stuff (pre watch 11) I put the phone in DND and everything is fine and I would check Pillow the next day for my sleep stuff - Now I notice if I put my watch into DND and choose Sleep rather than DND the hourly notifications come through..... I do note the apple watch does seem to still see my sleep overnight but this is sleep mode apple why would i want haptic buzzing every hour on my wrist .......

Check your sleep focus Settings, the app you’re using likely has notifications allowed.
i do not get any notifications while in sleep focus

Check your sleep focus settings as you might have notifications enabled. You need to tweak it to your liking or else your watch / phone won't perform the way you wanted it.

It’s not an app it’s built into the watch in accessibility as an hourly chime....seems a big oversight to allow its own built in system to over-ride the sleep option and give me haptic feedback every hour when I am meant to be asleep with no possibility to change except turning off the chime every night...

I see, so it seems like the accessibility feature overrides the focus setting.
I am not good at automations/shortcuts, but could you use an automation to turn the accessibility feature off when sleep focus is on and on again when focus is off?
I'd also contact Apple

Maybe layer a scheduled DND from the phone? I have that set for a slightly longer timeframe than sleep schedule and am not,bothered by hourly chimes or anything else.

I am experiencing a similar problem. I started using Sleep Tracking with my new iPhone 16 Pro and Apple Watch 9 after reading about the Sleep Tracking improvements in iOS 18/Watch OS11 (which the watch has been upgraded to). Every night my phone and watch go into Sleep Focus at the scheduled time, yet I receive Stand reminders (i.e. via the watch's haptics system and a sound - the same alerts I get during the day when I appreciate the reminders), two or three times a night. I also received one notification of a credit card charge having gone through which probably came from a third-party banking app.

I have had two phone sessions with Apple Support to try to address this problem. Both support advisers agreed that this is not expected behavior, and have confirmed that I have Sleep focus set up correctly. But other than stumbling around in the phone's and watch's settings, they haven't seemed to know what the source of the problem is or how to fix it.

I am literally losing sleep over this problem, so I would really appreciate it if any users have concrete suggestions.

If you mean Settings>Focus>Sleep>Choose Apps on the iPhone, there are no apps listed. There is a "menu" offering "Silence Notifications From" and "Allow Notifications From". The latter is checked, and as I said above there are no apps listed. Selecting the "Allow Notifications From" option displays the explanatory text: "When Sleep Focus is on, notifications from apps you select will be allowed. All others will be silenced and sent to Notification Center." Given that I have not selected any apps for this setting I would expect to not receive any notifications.

Going to Settings>Focus>Sleep on the watch does not offer any options to affect notifications (or anything else).

While working out today it occurred to me that because I have a Fitness focus set up, I do not get notifications other than in regards to meeting my workout goals on the watch while I'm in a workout. So clearly the watch has the ability to filter what notifications it activates. That helps confirm my suspicion that this is a bug with Sleep focus.

I’ve had the same experience as dbdbdb, it started maybe 1 month ago (?) so I have suspected it‘s related to the iOS update, or WatchOS or both. It’s quite strange and annoying, sometimes just 1-2 times a night, but since I am sleeping I don’t know 100% if and when the “stand” reminder comes, for sure. But it does happen at random during the night and before 1-2 months ago, it was reliably silent. I did add similar posts to apple.com/feedback, who knows if they read that stuff or not.

I should say that the Sleep mode and Do Not Disturb features are VERY appreciated, overall, since I get up early and go to bed early, it’s a godsend to be able to “set it and forget it” re: text notifications, calls, etc. the Stand reminders is definitely weird though, not a huge issue for me but I hope they fix it soon.
