I found the model of T-mobile iPhone 6s is A1687, same with Verizon's model.
    So, is the pay in full T-mobile iPhone 6s bought from Apple Store Online unlocked to CDMA EV-DO Rev. A (800, 1700/2100, 1900, 2100 MHz) & TD-LTE (Bands 38, 39, 40, 41)?

The T-Mobile phone will be fully compatible with Verizon's LTE and CDMA networks.

Yes. T-Mobile iPhone6 proved it. I believe the same thing for iPhone 6s. The onlything unsure is that if it support TD bands.

Supports everything but At&t LTE 30 band. Otherwise the models are exact same.

So is T-Mobile 6S unlocked out of the box if you pay full price through Apple or does it need to be activated on T-Mobile first?

It comes unlocked. Take out the tmobile sim-card before starting up and pop in whatever you have and you're good to go.
