The screen itself won't scratch easily (those real scratches you can feel with your nail), even with keys or coins). If you drop it or it slides on a concrete floor or asphalt it could get scratched due to quartz particles (which are harder than gorilla glass) on the floor. (If it doesn't break)
With normal use you don't have to worry about scratches. I've had my iPhone 5 naked for a year and have no scratches on the screen.
Are you sure? None? Not even a tiny one that you can only see if you hold your phone up to the light at a certain angle?
And how do you usually carry your phone around?
No scratches. No screen protector. Never plan too. I leave my 5s naked.
This. Once upon a time I got my kids some used DS Lites. The screens were scratched and they cracked another. In operation it was no big deal; defects were not noticed.My iPhone 4 had several real scratches on the screen (you could easily feel them), but they were invisible once the screen was on, and didn't impact using the device at all.

Having said that, I prefer to run my iPhones with a super slim case and screen protector. I like the extra layer of protection for those oh-54!7 moments.
BTW: No scratches on my screen
