iPod Anyone have Student Union Rebate Form?

I know, it blows. It's day 46, and I won't be able to get my iPod UPC till tomorrow anyway.

I've sent in another unrelated rebate a few days late before and it was rejected.

upc label for rebate

hi can anyone clarify which label is the upc label they need to process the rebate? i know it's on the carton the stuff came in, but there are two labels with barcodes on them. it says they need the one with, the part number, the serial number, and the product description. but the one with the part and serial #'s has no product description and the one with the product description only has the part number, no serial number. any ideas?

The rebate form should have an image of an example UPC code. I just followed that: it's a relatively large 'square' (for the computer UPC)-- roughly 3" x 3".
