I did a little sleuthing around (right clicked your image and "Search Google for this image") and came up with;

Simplicify is the name of the theme.
Supported on iPhone 5 and iPhone 5S only.

A quick look for it on Cydia pulled up the theme. I haven't tested it. Does say it's designed for IOS7+ (not sure about 8) and requires LockHTML or Groovylock.

simplicify is on cydia, works great here on my 5S...(using it with lockhtml4)...
