I'm running into an issue, I think.

My apps are set to auto update but they don't. An example is my Yelp app. There was an update released but it never updated on its own. When I went to look for the update, I had to search for the app and on the search screen the update button had a "+" in the top left of the button. I updated it manually and then went back and it's still there. Here's a screen shot.

It's even on apps that I haven't installed before.

What does that symbol mean?

Also, why is it so persistent to not update the apps in the background even though under settings and iTunes App Store, it says it is set up auto download apps.

Very strange.
 
My iPhone 5s running iOS7.0.2 is auto updating fine. However, there is a lag from when an app is updated and when it auto updates. This should be the case, otherwise the phone will be polling the App store constantly and draining the battery.
 
Auto updates only check in once in a while, and with the App Store not being really instant anyway (as it hasn't really been for most of its existence), you can likely expect a "delay" of getting updates up to a day or so after they become available.

The "+" you see in the App Store simply means an app is a universal one (for iPhone/iPod and iPad), and doesn't really have anything to do with update availability.
