I have an iPhone 12 Pro which I got on release day 2 years ago. I decided to pay monthly rather than one time purchase as I had lots of iTunes store credits which I purchased at a discount, so it worked out pretty much the same.

Question is: My plan renews tomorrow for another month. Does it allow to continue coverage as long as I keep paying? I have some weird problems with my cell connectivity which could be hardware related. I tried a SIM swap to see if that resolves the issue before contacting Apple. Also battery health could hit 80% soon so that would entitle me to a replacement. Wondering if there a time limit on how long you can have AppleCare for?

You can continue the month-to-month beyond two years. Actually, I think I was able to add a month-to-month on an old XS Max a couple years ago (which I did for about 4 months). I think Apple is happy to take the cash (of course) and I don't know if there's an end to how long you can extend it.
 
I think you can subscribe to Apple for up to 3 years.
Nah you can do it beyond that (basically indefinitely until you cancel at least in Australia, Canada, France, Germany, Italy, Japan, Spain, the United Kingdom, and the United States):


Your plan coverage may continue beyond 24 or 36 months, on a monthly or annual basis, until canceled.
