Ran a routine AHT Apple Hardware Test on my 5,1 firmware upgraded Mac Pro 2009 that works fine and got this error:

apple hardware test error 4vdc/1/40000003:videocontroller

As I am having no issues I guess I can just ignore it.

Its running the stock GT120 that came with it.

Any suggestions welcomed thanks.
 
I ended up downgrading the firmware back to 4,1 from 5,1 and AHT now shows no errors. Hey! Hey!

Was quite a fiddle to get the downgrade to work - had to download and rename both firmwares so the tool would work, and also disable SIP.

I just upgraded my Early 2009 Mac Pro from 4,1 Quad Core to 5,1 6 Core, original GT120 and got the same error.
No issues.
