Apple's rules are guidelines. There is no specific legality assigned to them. Apple are free to ignore their own rules completely if they so wish. Their discretion is absolute.

Apple has ignored their own rules many times before, and sometimes had to change them.
One that comes to mind is when they banned a satire app, and then got ridiculed themselves because the author was a Pulitzer Prize winner.
What really kills me is this: Steve Jobs used to make fun of carrier "orifices" that controlled what apps people could use and buy. As it turned out, he simply wanted to make his own walled garden, only higher than the carriers, because at least they allowed smartphones to download apps from anywhere.

In some cases, the engineers and the people providing the images and text are not the same people. It can be marcom people preparing the images and description for app submission.
With that said, It is easy to lose track of such content for news feeds and such.
I think this might just be an over-zealous approver. Someone that can't think outside the box, even a little.
Finally, I'm not sure how fast it goes for a small correction to artwork, but for the apps I work on, and update is about a week waiting time. Hopefully, it doesn't take another week to just change the meta-data.
