This past MON (10/22/2018) I completed a 2,500 vertical foot hike with both my iPhone X active (using the Cyclemeter app, image below) and my Apple Watch 4 in Workout mode, and in Airplane mode as well to conserve battery life. Both devices/apps correctly recorded distance and heart rate. However the Activity monitor only showed 38 flights (2nd image below). It should have been closer to 250 flights. Any ideas what is going wrong? I have noticed the same issue on multiple other days.

the only thing I can think of is if your have more than 1 apple watch and have automatic watch detection turned off and you are wearing a watch that is not connected to the phone.
 
Works off barometric pressure. This might have been changing while on your climb, skewing your results. I would not get too hung up on exact numbers. My watch indicated I climbed over 30 flights or stairs while having a hurricane go by.
