I have an iPhone 12 Mini on 02 SIM mobile Network in the UK. I am looking to get an Apple Watch that I can use as a 2nd phone with a Czech SIM card, is this possible?

So I would have 2 phone numbers, one in the Uk and one in the Czech Republic. UK number goes to iPhone and the Czech to the Watch?

Then pairing them if possible...?
 
At least in the U.S., the watch does not have independent phone capabilities (i.e., it is not possible to use it for calls without those calls being essentially routed through the paired iPhone).

Apple Watch does get it's own number, for example with AT&T the watch has a separate 'phone number' from the iPhone, but this is only to differentiate the watch for their billing and account management purposes. It isn't a 'phone number' that can be called, separate from the phone number of the paired iPhone.

Can always have the O2 setup for eSIM and then slide in a Czech SIM card into the phone for the second line.
Exactly, you can have both the UK number and the Czech number in the iPhone and the UK number paired with the Apple Watch. You can have both the UK number and the Czech number in the Apple Watch too, but only one will work at a time, and only in the country where the original carrier is, as it doesn't support roaming.

So you can have UK number + Czech number roaming on the iPhone + UK number active on the Apple Watch while in UK, and UK number roaming + the Czech number on the iPhone and active on the Apple Watch while in Czech Republic.
