What happens if one were to break due to accidental damage, after having repaired it twice, exhausting AppleCare+ claims for accidental damage? Can you buy AppleCare+ again? Is it now not covered for accidental damage at all? Thanks for any insight you may have.

Above is correct. Once you used up two AppleCare plus to repair your phone, then you will have to pay the out of warranty cost to replace/repair your phone. Once you used up two instances, it cannot be renewed. Period.
