a) If I buy an iPad, iPhone, Watch, respectively from Amazon (their 'main' offering which comes up searching for that product; usually eligible for Prime shipping, Fulfilled by Amazon at minimum; not Joe's Garage Shop selling on Amazon), where can I get AppleCare Plus, within 60 days? Call Apple on the phone (they run diagnostics - technically, how? it's interesting) and I check out by phone?

Interestingly Amazon started selling (not sure how long ago) refurbished iPhones, for example: http://www.amazon.com/Apple-Unlocked-Smartphone-Certified-Refurbished/dp/B00YD547Q6

Discussing such advanced topics with Amazon directly is pretty painful, though.

b) Even if I buy the Watch, iPad from another reseller like B&H Photo, Best Buy, I can only buy the AppleCare Plus from Apple, on the phone, not from the reseller?

Well, your best source of this info is not this forum, just go to Apple.com, select any product (iPhone, iPad, watch, whatever), and on product selection page (like when you select the storage for iPhone), there will be a chat bottom. Click it and ask Apple about how you can buy Apple care. Their answer is the official answer.
Remember to save the transcript, so even if there is something wrong, you can use the transcript to talk to Apple, they will usually make an exception for you if you were told so.

Wherever you buy Apple product you can buy AppleCare+ (within applicable time period and terms and conditions) very simply and quickly by phone call to Apple. It takes 5-10 minutes: few questions to confirm your AppleID/details and running of diagnostic on device to be covered.
