if I get the phone financed through ATT Next and buy Apple Care seperately, which i believe covers for 2 years, and upgrade the phone after only one year does the plan transfer?

I think no

The plan stays with phone, but you can get a refund for the unused amount. So after a year you would get a refund of $65.

You do have to call them though... it's not automatic.
