I currently own about 10 different iOS iPadOS devices and have automatic system updates enabled on all of them. I can't remember the last time one of these devices actually updated on its own.

There will come a point where it will require your device passcode (if you have one) to preapprove a pending update. After which point it will update once certain conditions are met (min battery level, time of day, on a charger).

My iPhone and iPad like to update overnight while charging, but my Apple Watch will update any time of the day as long as it is over 50% and on the charger.

The auto updates only work overnight while it’s charging and connected to Wi-Fi. Are you doing that? I’m guessing it also needs to know your regular usage pattern, so it knows when a good time to update would be.

My parents each have an iPhone and iPad. Their devices are always up-to-date whenever I’m home to visit them, so it’s been working for them. They have a regular habit of charging their devices overnight when needed.

As for me, I usually update my devices right away when an update is released, but they have updated on their own when I’ve forgotten.

I keep Auto Updates turned off due to the buggy garbage we have with new updates lately. But when I did allow it, I don't think it ever worked correctly. Plus even when the device is plugged in at night and idle, the most it will do is download the update file, you still have to enter the pass word to authorize it.

Mostly works like the app store on most devices. 7 days after the update the auto update will work. Its annoying, especially when I get a message "iOS xxx is available, your device will update tonight". I mean it could have just done it instead of displaying the message but yeah whatever you do that iPhone....
