On iOS 10 on my 6s plus I am losing battery because Waze and SiriusXM are doing background activities but I have background refresh turned off? What's going on

Do you have the apps open? If you don't, swipe them up and fully close them out, they will still run in the background. Background refresh refers to updating when the app is fully closed out.
They can also be performing some background activities for a bit after you use them. Background refresh is just one aspect of apps doing something in the background, the general ability of being able to do something background is built into iOS and isn't controllable by the end user.
