iPhone XR Battery drain due to background use

Recently I have seen greater battery drain due to background usage, as below:

I haven’t changed any settings so no idea what is going on. Could it be to do with the below?

Turn every background process off. It doesn't really help and def takes a toll on the battery

As in background app refresh? Why does it not help?

I haven't changed any settings other than turning on icloud photos so surely that has to be the culprit? I've used it for a year with background app refresh turned on but never had this much background battery drain.
