If you know your battery qualifies, don't even bother with the webform. I suspect it was intended for the initial rush, when may people were making claims. Call Apple directly. Tell them you used the form a month ago, and are disappointed with the lack of a response.

Otherwise, be polite, and they should at least assess the situation.
 
Yeah, as the FAQ says, you should received the battery in 3-5 business days. I'd give Apple a ring and see what's up.
 
A call into Applecare will get it taken care of. I don't know if it applies to everyone, but they had me ship my original battery back in the packaging the new one came in (it was prepaid).

They shipped it one-day as well.
 
There's no Apple Store near me in Northern Ireland, though there is one opening in August-ish time.

Just call them...
Often the battery is counted as a DIY item (user installable), so Apple should be able to send you a replacement via the post, though they might request the old one back - usually depends on the branch/country.
