I think no gift is better than your presence and care to her. Besides Mother's day, we should spare more time to accompany our mothers. :)

and with this, I would say that the best gift would be a trip to see your mother, then take her on a trip to see her mother (read: your grandmother).

What makes that even better is that if you have children of your own, it's a rarity nowadays if you can get a 4-generation photo; even rarer if your great-grandmother is alive, which you can get a 5-generation photo.

I used Ink, a service a bit like Apple's discontinued 'Cards', to send a childhood photo as a card.

I just get my wife whatever she asks for.

Eh, that's what you get for marrying your mother. At least you probably get along with the in-laws.

1) a nice family gathering
2) someone does whatever "tasks" she normally does for that day
3) an outing of her choice (restaurant, movie etc.)
4) usual small gifts (including chocolates, favourite flowers etc.)

and the high end -

5) spa day followed by outing/restaurant (or a massage)
6) jewelry
7) high end restaurant

For some of us - laying down flowers, reflection and a simple Happy Mothers Day RIP under our breath
 
This year, I got my wife a fitbit. A bunch of her friends/family recently got one and she either was really subtly hinting at one, or was genuinely asking questions about it. Either way its sitting in an amazon box on my counter, ready to be wrapped :)
