Turned Keychain off/on and deleted data from 5s
Logged back in, approved device and still not working......😡

Try turning iCloud Keychain off and the go to "Settings" -> "General" ->
"Reset" -> "Reset All Settings".

-NOTE this will take some time to restore all your proper settings back to your personal-liking but it should fix your problem. Let me know.

just did the steps you mentioned and still nada.......
for example, if I load my banking site within safari, the user and password shows up properly to begin, but if I remove the user name and try to change to another user name the fields do not populate the info like it does in Safari on OS X
also, when I go into settings / safari / passwords & autotfill / saved passwords it lists all my active keychain accounts which is a good thing, just wish it would populate the info which is saved.

Yeah, seems like it might be a problem between OS X and iOS for iCloud Keychain.

when you say that, does that mean it is not working for you either?

I don't have a Mac (wish I did). So I am figuring that is the case though. I hope someone else with OS X and iOS looks into your issue and finds some sort of solution.
