Clean install so nothing. Seeing this on two different devices so thinking it may be the FileVault post-login screen running as an appIt doesn't normally happen on a Mac. But we don't know what software you have installed. Check your login items in System Settings.
What device and version of macOS are you running? FileVault enabled or disabled?That definitely does not happen on mine. You see the icon in the Recents section of your Dock. Does it show in the Recents menu? If not, you'd probably have to go looking in the system logs.
