I have this app/process named BlueTool.

    What is it?

This is part of my crash system log

    deny mach-lookup com.apple.system.notification_center

    Process: BlueTool [165]
    Path: /usr/sbin/BlueTool
    Load Address: 0xfb000
    Identifier: BlueTool
    Version: ??? (???)
    Code Type: arm (Native)
    Parent Process: launchd [1]

It is also under Library /Application Support.

And what is this -> FacebookAccountMigrationDialog.app

    Is it by default?

Did you jailbreak your iPhone? The evasion jailbreak causes these Bluetool crashes, as it tampers with essential system processes.
