I noticed an issue with my bluetooth connection to my car's bluetooth system for phone calls. When I make a call there is no issue, the call goes thru my car's bluetooth handsfree system. But when I receive a call the phone is defaulting to the iPhone instead of the handsfree system, so I can't hear the caller and the caller can't hear me. I have a Prius V.

I was playing around today and figured out a workaround if anyone else has the same issue. If you go into the Accessibility settings on the iPhone the "Call Audio Routing" was on Automatic, I changed it to Bluetooth Headset and it now seems to work correctly. I never had to mess with that setting before.

I have this problem too (Renault R-Link system). If you have it set to Bluetooth Headset, what happens when you get an incoming call whilst away from your car? Does it then work as normal (call through the phone, not routed to a non active bluetooth device)?

Yes, it works completely normal away from the car, with the call going completely to the iPhone. It even works normally if you are connected to a Bluetooth speaker.

Thanks was having the same issue , if I remember to answer the call Thur the car steering wheel control it goes Bluetooth , but if I forget and answer on the phone it would go quiet unless I picked the phone up.
So I followed your tip and that should correct that problem
