I was having a permanent crash on boot, on Windows 8.1, after installing the Boot Camp drivers v5.1.5621. I've found a solution to this problem; I'm posting here, in case, someone has the same issue...

    My System:

    Mac Pro 2012
2 x 2 TB hard drives (software raid - mirror) OS X Mountain Lion
    2 x 2 TB hard drives (software raid - mirror) Windows 8.1

    a) OS X can't see or access Windows partitions (dynamic volumes) no crashes
    b) Windows crash on boot after boot camp drivers install. Remember, Windows volumes are dynamic!

    Solution:
    Install Windows 8.1
    Install BootCamp (do not reboot!)

    Browse to C:\Windows\System32\drivers\
    Delete or rename AppleHFS.sys & AppleMNT.sys

    Optional:
In the Start Menu search box type "regedit" and press Enter
    In Regedit delete:
    [HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Services\AppleHFS]
    [HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Services\AppleMNT]

    Reboot the system
Configure and create you're mirror set and let it synchronize
    Enjoy you're Mac
