Should pop up a reminder on your watch. Unfortunately I'm without mine until the new one arrives so I can't dig much deeper into why it's not working for you. Hopefully someone else will come along.
 
Seems to be working perfect for me. I hit snooze when it's inconvenient for me to breathe (LOL - I know - but you know what I mean). And it does remind me later.
 
I have turned off all notifications for this and I still get notified every 4 hrs. I want it to stop but it keeps coming.....
 
I set up the Breath app to remind me every hour but a reminder is not coming up, how is it supposed to remind you? A notification?

Same problem, reminders not coming up, I even start it each morning, hoping it will start reminding me, but it doesn't. Then sporadically it does remind me, about 5PM or later.

"We have new requirements for Breathe Reminders in watchOS 4: - user must be sedentary for a period of time - user must not have an event scheduled in their Calendar - if a user does a workout or another Breathe session, the next Breathe reminder will be rescheduled."
