Bug? Blank windows appear when I close a document; they cannot be closed

Is this some sort of Mountain Lion bug? Suddenly, occasionally, when I close a Preview window, another window of the same dimensions—but blank—manifests underneath the closed one. I cannot close it, and have to force quit the application.

A similar problem happened yesterday with Mail; there was an all-black window (with no title bar or anything).

Why is this happening?

I do not know why this happens but I'm wondering whether you restarted your Mac in-between and whether the same things are still happening then.
