Same with T-Mobile. If you've met your committment they will unlock for you free of charge. If you own a Verizon iPhone 5 or higher then your phone is already fully unlocked. If you own a Verizon iPhone 4s or below OR are a Sprint iPhone owner, you are totally out of luck. There is no domestic unlock for these. If you are using a foreign carrier then I have no idea what you'd need to do for that. I'm being thorough because you didn't mention your carrier or your model of iPhone.

Sure does but you can pay for an imei unlock or contact the carrier that its locked to in order to request them to unlock it. Depending on what model iphone and what network its locked to of course.
