Anyway, there are literally hundreds of millions of iOS users, and many of them use their devices for quite a long time without having ergonomics issues and without requiring a mouse. You should probably observe how they do it, and modify your usage accordingly.
 
A Bluetooth keyboard is possible, so a Bluetooth mouse/trackpad is at least theoretically possible - USB need not apply. When I'm writing (professionally) on my iPad, I've often wished I had a mouse to supplement the Bluetooth keyboard - reaching up to the propped-up iPad screen is not optimal as far as I'm concerned.

Would Apple add iOS support for pointing devices? I wouldn't put money down on it, one way or the other. Siri support for pointing/selecting seems a safer bet, as it would be an accessibility feature as well.

Now, is a higher pixel density a good reason to favor a tiny screen over a substantially larger screen? Not in my book. Talk about bad ergonomics! Eye strain is a much bigger issue for me than arm- or neck-muscle strain.
 
Unless you have a physical condition that prohibits you from lifting your arm, I believe that the rest of us here deal with it every day.

Same place you find those folks that wave a big fan over your as you recline on your chaise longue. Get a grape-holder too, while you're at it! :)

May not be helpful... But if you really can't stop holding your phone, maybe use it as a mouse itself. I know it works as a mouse for most computers, not sure about iPads though. May have to be a jailbroken iPad for it to work but not 100% sure.
