Just recently I was going to use my iPhone to pay for dinner at a fast food. At first it said I couldn’t use my card. So I remove them from wallet. Reintroduce it and it won’t add. Appears to be related to this latest 26.3 update? Sounds like a bug….
 
I think it’s a bug issue with the iOS.
Maybe...maybe not... It's been happening to people ever since Apple Pay was introduced. It'd be weird if it was a bug that hasn't been fixed after all these years.

Here's a thread from last year, where supposedly that person's device's trust score was low.

Even Apple says to contact the card issuer if you have issues, especially if you're getting one of the errors their support article mentions. The card issuer is the only one that can tell you why it's not working.

 
This is the first time I had an issue with it. Only recently. The Mastercard from another bank. My debit/credit card from one bank is the issue. Was not aware this has a ongoing issue.

What error message appears when you try to add a card?

As others mentioned try calling the number on back of card as bank may be blocking for some reason or random security error on account. Rare but I've had a card not add and bank was able to verify and clear it.

This happened to me before - I factory reset my phone and 1 card just couldn't be added back.

All I will say is good luck, because the bank kept telling me that the problem is on Apple's side and Apple kept telling me the problem is on the bank's side. After a month of this back and forth, the bank ultimately caved and sent me a new card with a new card number.
 
Wife and I both had cards. Joined them into co-owned with family share. Card showed on her phone but disappeared from mine and couldn’t add back. Case escalated in engineering for a year then got closed somehow. Escalated it again through Exec Relations and two days later I updated to 26.3 the day it was released and I all of a sudden got it added. Apparently an old problem that still exists.
