I am running 10.11 on my Mac and 9.1 on my iPad but I still see the old version of Notes.

No matter what I try to do the new version never seems to load.

What could I be doing wrong?

The new notes only seem to work for notes created on iCloud rather than notes enabled on other accounts you may have connected. Make sure under your iCloud settings notes are enabled - you'll then see that option listed on the notes app when viewing the list of folders.
 
Is the note being created under the iCloud account? It's not defaulting to another is it?
Aside from that I'm afraid I'm not sure what else to suggest :(

iCloud on the Internet is working fine. It's on the iPad that the new version isn't showing up. It's a first gen mini but it's running 9.1. I thought all you need to do is to run 9.0 or above, but maybe it's the iPad blocking the update.

Have you 1) enabled Notes in iCloud on your iPad, and 2) created a NEW note on that iPad? Existing notes will not show the new tools, and only new notes that are stored on iCloud will show the new tools.
 
Looking at your iPad notes folders - do you see an iCloud option like this? If so select the 'Notes' folder under this and try creating a new note.
Does the resulting note have the new options?

I found the answer. Apparently the sketch feature is NOT available across all devices. It can only be used in iPad minis from the Retina model on up, and in the full sized iPad from the iPad 4 on up. Apple really dropped the ball on this one. From all of their other descriptions about iOS 9 they made it look like all you needed was to update to iOS 9 and all the features would appear, but that's not the case. This was the only feature in iOS 9 that I was really looking forward to using. Very disappointed to say the least.
