I received an online exchange from Apple Care for my 6S+ in a white box. During setup it asked me to pick my carrier from a list of US and overseas service providers...usually it just selects AT&T and activates my phone.

Is this new? Is my phone possibly unlocked?

the store replacements do the same thing. if you picked your carrier, more than likely it also locked itself to that carrier, unless you have T-Mobile.

One more thing, I had to reset it today and it asked me again. Does that make a difference?

no, doesn't make any difference. it might still be unlocked though. What carrier are you currently using? You can always select T-Mobile when it asks you, and it should remain unlocked.

Using AT&T. I will try another SIM and see if it works...that would be pretty cool if it did.

Yeah I noticed that on a replacement I just got too but don't have another sim to test it out.
