It’s the same as FaceTime, and walkie talkie, in Dubai it’s not available on iPhones and Apple Watch. Apple somehow disables these functions. Would like to know how, as the iOS updates are the same I assume as I occasionally update in the USA. Or the updates are hardware specific. Would love to figure out a way to enable FaceTime on my Dubai iPhone X. My Apple Watch 4 also did not get ecg with the update.
Apple Pay is not an over the counter medical device.
I’m not sure how it works in your country, but in most places there are different regulations that govern banking and healthcare.
All they have to check is the serial or model number. In those numbers, Apple does include identifiers for the different regions where they sell devices and that's what they seem to be using to enable or disable ECG on the Apple Watch.It would not be difficult for the update software to ‘phone home’ and find out where your equipment was purchased before loading parts of the software.
Pay for something you cannot use even if you moved to the US because your watch was purchased elsewhere. Ridiculous. Doesn’t make sense.

No surprise here. Apple's lawyers are already pretty backlogged dealing with Qualcomm and iPhone and MacBook class actions. Enabling ECG worldwide is just going to cause even more workload.
From a trillion $ Company? Naaah. Its more likely the clearence from individual countries that’s the bottleneck.

If Apple followed your suggestion and waited to release the ECG in the US, more people would be at risk of suffering from undiagnosed atrial fibrillation.Like I said before, government approval takes longer than agreement with private entities such as banks. If it took Apple two years to start rolling out Apple Pay to EU countries, it will take them even longer to roll out ECG. Don't expect to see it there before 2021. After previous feature rollout experiences, apple should have known better and should have waited to release ECG in the US until they had approval requests submitted in a few other countries. That way people abroad wouldn't have to wait as long as they will have to after the US release to get the feature.

So this begs the question: can I just buy a series 4 while in the U.S. (I'm Canada based), bring it home and enjoy the feature? We used to do this with the original iPhone before Canada got them and have done so with lots of tech stuff in the past. If the unit itself is a U.S. model and I set my region to the U.S., which I have already done to get the Apple News App, will I be able to use the ECG feature? Might have to investigate this one with a trip south!

The only right choice, being a health feature and knowing government approval takes time, was for apple to have put in approval requests in other countries AT THE SAME TIME WHEN THEY SUBMITTED THEIR REQUEST FOR APPROVAL FROM THE FDA IN THE US. That way approval in other countries could have been obtained sooner after the US release if not even at the same time in some places. Instead, Apple only requested FDA approval in the US and didn’t even bother to try anywhere else, and still hasn’t. Now millions of people around the world are still going to be at risk of suffering undiagnosed atrial fibrillation for at least another two years because Apple didn’t bother to request approval from the health entities in their countries earlier.If Apple followed your suggestion and waited to release the ECG in the US, more people would be at risk of suffering from undiagnosed atrial fibrillation.
There are already reports of the feature "saving" people in the US, so I think Apple made the right choice.

Luckily i got a friend of mine to get my series 3 from the usa when he was on holiday and bring it back to me in the uk. So i should have ecg now!It's based on the law, which is where the product is sold.
The FDA only gave Apple clearance to sell it in the US. The FDA did not give Apple clearance to sell it to US Citizens in the UK, for example.
It needs to be the series 4 though, because ECG requires specific hardware that only the series 4 has.
