Apple website (https://support.apple.com/en-gb/HT204666) states this:

"HRV measurements aren't available in China, Malaysia, South Africa, and Turkey."

I've got an Apple Watch S2 from China on watchOS 4 and indeed I cannot get it to measure the HRV. With that said, I'm neither Chinese nor am I based in China (or any of the above countries). I'm in the UK and the watch is set up with the UK locality so I'm puzzled as to why I'm not getting HRV measurements. Are Chinese watches physically different, i.e., is there a piece of hardware missing from them?
 
No, I am 99% sure they aren’t. Apple is imho too lazy and profithorny to do such a thing. (Which is a good thing imho)

Two possibilities:

1. Its region based like Apple Pay not showing up when choosing German language (fml). Switching to <insert compatible country here> and then it will work tho

2.1 it’s based off your iPhone, means where your iPhone is bought from you might have some restrictions.
Like no FaceTime in SaudiArabia, EU volume in EU countries, Force Shutter Sound in some
Asian countries es etc usually then if you use a non Chinese phone it would work. (Though you can change the file with jailbreak. Here a small documentation of mine: https://reddit.com/r/jailbreak/comments/7lhb3k/tutorial_ios_1112_remove_regionrestrictions_like/)

2.2 the same as above just that the file is on your Apple Watch. But then I guess you can just call Apple and tell them your crown stucks or whatever, and they will replace it. Or ask them to downgrade your watch (that’s what a friend of mine did cuz he downgraded his phone from
iOS 11.3 Beta to iOS 11.2)

They will probably just replace it with another identical "Chinese region" watch. The type is identified when you take it in for service.

I've got the region set as the UK and language as English on the Watch. Same settings on my iPhone 7, which was bought in the UK. So it isn't purely settings related nor it is tied to where the phone was bought - at least not in my case. I was hoping a bit the iOS 11.3 / watchOS 4.3 updates would have enabled HRV, but no dice.

Could you have a look at what model the watch is for me please? Watch app > general > about > Model. Mine is MNNW2CH/A. CH for China I suppose. I wonder whether Turkey has such a model or perhaps if for Turkey it is settings related?
 
HRV is considered a health measurement which is normally measured by ECGs. There’s a requirement in China for clinical trials to prove a medical device’s accuracy. I’m guessing Apple chose not to or couldn’t pass the trial.

One of the main reasons I purchased my AW is because of the HRV. If it didn't have HRV due to some geographic setting I'd be pretty pissed tbh.

I have the same issue never been able to get the HRV to work (series 1).

I purchased the watch in Europe but the serial number has SO and that is South Africa according to a serial numer country list I found !!!

My wifes AW HRV works but her watch was purchased in the US and has the LL in the model number.
